- Have all the boxes been completed? _____
- If the number of credits is variable, does the course description specify what is required by students for the various numbers of credits? _____
- For upper level courses, are there prerequisites (at least "permission of instructor")? _____
- Are the course times legal? _____
- Course description: Will it make sense to the target students? Is it grammatically correct? Is it 50 words or less? _____
- Have all the revisions been listed in the "For Revisions" box? _____
- Has the instructor sufficiently determined and described the overlap with other courses? _____
- If request is made for college distribution, is there a statement of justification and a course syllabus? Is the instructor's CV included with the proposal? _____
- If cross listing is requested, is there a memo of justification signed by all the chairs of the departments/units involved? _____
- Is the proposal signed by the CCC representative and department chair? _____
- Proposals for new ("add") courses need to be accompanied by a one-page list of topics for the course. _____
