Independent Study Forms can be picked up at the CALS Registrar's Office, or downloaded and printed off from clicking here. This form can only be used for an individual study, supervised teaching, internship, or undergraduate research course in the College of Agriculture and Life Sciences. This means a CALS faculty member must be responsible for assigning a grade. This form cannot be used for BIO G 209 or BIO G 499. Please contact the Biology Department directly in 216 Stimson or visit their web site at http://www.bio.cornell.edu/forms/ for more information.

Application to graduate early/delayed/elsewhere
Students under extenuating circumstances can apply to graduate early, delayed or elsewhere. Students can download the form from clicking here, or they can come to the CALS Registrar's Office (140 Roberts Hall) to pick up a form. It will need to be filled out by the student, signed by the advisor, and then brought back to the CALS Registrar's Office for the Registrar's Approval.
Effective Spring 2005, CALS students may repeat a course previously completed at Cornell. Students may wish to repeat a course in order to demonstrate an increased understanding of course material or to improve a grade. If you wish to repeat a course, please submit the Repeating a Course form to the CALS Registrar’s Office in 140 Roberts Hall. This is for record-keeping purposes only.

Authority for Release of Academic Information
If a student wishes to receive Dean Certification, they must fill out a form permitting the Judicial Affairs office to release student disciplinary information in order to complete the Dean's Certification.

DUST allows students to view their academic information, as well as advisor's to view their advisees academic information (degree progress, unofficial transcripts).For the most up to date information on your CALS distribution requirements please schedule an appointment with Registrar Staff, or come to walk-in hours.
Use your net ID and password to order a transcript online from the University Registrar.
